Four Month Licensure by Endorsement

The Four-Month Licensure by Endorsement Program is designed to prepare experienced maternity care providers, and foreign Doctors for careers as Florida Licensed Midwives. The program focuses on the art of out-of-hospital birth, equipping students with the knowledge and skills necessary to be a safe independent practitioner. Course work is both academic and clinical, with a strong emphasis on the laws and rules that govern practice as a Florida Licensed Midwife. The Midwifery Endorsement curriculum consists of 25 credit hours and 450 clock hours.

In accordance with Florida statutes, licensure by endorsement students must perform ten supervised prenatal examinations, and act as the primary midwife at five supervised births. The program is open to practitioners licensed under statutes other that F.S. 467, and foreign trained practitioners that have had their credentials reviewed and approved by the appropriate state agency.

After approval from council, students are prepared to pass the Florida Licensing exam/North American Registry of Midwives (NARM) exam. Upon passing this exam, graduates may practice as Florida Licensed Midwives.

NB: Graduates of the Four-Month Licensure by Endorsement Program will receive a Diploma.

DEPARTMENT OF HEALTH COUNCIL OF LICENSED MIDWIFERY APPLICATION CHECKLIST:

Within 30 days from receipt of your application and fee you will be notified via mail of your application status. If you do not receive notification within 40 days from receipt of your application, please contact our office. Documentation Applicants Must Submit:

  • Complete the Confidential and Exempt from Public Records Disclosure Form.
  • Completed application, printed (clearly) or typed
  • Cashiers Check or money order made payable to the Department of Health.

Determination of Eligibility for 4-Month Pre-licensure Course

  • Application Fee $100.00 (non-refundable)
  • One full-faced passport type photograph (size 2X2) taken within the past six months
  • Name change documentation (if applicable)
  • Clarification of deviation in identity as it pertains to submitted documents
  • High School Diploma or Equivalent
  • Official transcript from an approved midwifery training program
  • All courses successfully completed

Official transcript from the midwifery program which documents classroom instructions and clinical training Another County:

  • Certified official certificate or diploma from a foreign institution of medicine or foreign school of midwifery AND a certified translation of the foreign certificate or diploma when applicable.
  • Certified copy of document rendering applicant eligible to practice medicine or midwifery in the country where document was issued AND certificate translation of that same document when applicable.
